Disorders of sexual development (DSD) refers to a group diseases that links the mismatch between an individual’s genetic and gonadal its phenotype. Ovotesticular DSD (true hermaphroditism) is one such disease, in which both male female gonads are present. A 15-year-old boy with history surgery for non-palpable testis was examined due bilateral gynecomastia known gonosomal mosaic Klinefelter syndrome. The external genital matured as and, left half scrotum, there testicle normal size. Despite uncertain resistance on right side, however, not palpable. Revision groin revealed surprising finding form ovary dilated fallopian tube, were completely removed. Surgical revision biopsy performed. completed mastectomy. postoperative course uncomplicated, content fully integrated into his peer group. True hermaphroditism rare type DSD. In case described, exhibited until puberty, after examination gynecomastia. also confirms necessity clarification long-term follow-up patients unclear findings during testis. Diagnostic laparoscopy clearly indicated these situations.
