Outcomes of intraoperative ultrasound for endoscopic endonasal transsphenoidal pituitary surgery in adenomas with parasellar extension

چکیده
Pituitary adenomas with parasellar extension present a technical challenge for adequate visualization and gross total resection (GTR). The endoscope improves identification of extension, however, additional intraoperative imaging adjuncts can further augment visualization. Intraoperative ultrasound (iUS) may provide viable cost-effective solution imaging. We sought to assess the ability predict extent on 3-month postoperative magnetic resonance (MRI) in pituitary extension. Twenty consecutive patients undergoing endoscopic endonasal transsphenoidal surgery assistance were prospectively collected. findings recorded during each case. MRI studies reviewed blinded fashion residual tumor compared findings. Median preoperative Knosp grade was 2. Cavernous sinus invasion encountered intraoperatively 3 patients, all whom preoperatively. operative time 152 min. Based iUS findings, 17 expected have GTR while underwent subtotal resection. 18 completed MRI. concordant 16 cases (88.9%) only two instances discordance. reliably as assessed by Image capture interpretation vary based operator experience. Ultrasound provides reliable immediate assessment resection, normal gland other important neurovascular structures.

Management of Pituitary Adenomas: Mononostril Endoscopic Transsphenoidal Surgery
Introduction: The endoscopic transsphenoidal approach for pituitary adenomas and other sellar lesions is quickly becoming the procedure of choice in their surgical management. The most common approach is binostril three-hand technique which requires a large exposure and subjects both nasal cavities to potential trauma. To reduce nasal morbidity, we employ a mononostril two-hand technique with t...
متن کاملEndoscopic Endonasal Transsphenoidal Approach for Pituitary Adenomas: a Prospective Review of Our Early Experience
Background & Aim: Pituitary adenomas are a part of a dissimilar group of benign neoplasms. The development of endoscopic techniques for surgery of paranasal sinuses has increased the opportunity for an endoscopic approach with regard to the pituitary gland. Minimally invasive endoscopic pituitary surgery, in turn, permits a more thorough tumor resection and fewer associated surgical complicatio...
متن کاملParasellar Extension Grades and Surgical Extent in Endoscopic Endonasal Transsphenoidal Surgery for Pituitary Adenomas : A Single Surgeon's Consecutive Series with the Aspects of Reliability and Clinical Validity
OBJECTIVE The inter-rater reliability of the modified Knosp's classification was measured before the analysis. The clinical validity of the parasellar extension grading system was evaluated by investigating the extents of resection and complication rates among the grades in the endoscopic endonasal transsphenoidal surgery (EETS) for pituitary adenomas. METHODS From November 2008 to August 201...
