Optimizing mapping at low-frequency hearing in electric acoustic stimulation devices

چکیده
要旨: 残存聴力活用型人工内耳 (EAS) の調整は, 残存聴力の程度により, 音響刺激の利得と電気刺激の下限周波数を決定し, 人工内耳と同様に THR と MCL を測定する。今回, 実際にこのような調整方法で上手くいかず, 工夫を要する例を経験したため, デフォルトの調整でマップを作成した例と比較検討した。その結果, 術前に補聴器を常時装用していた症例では, 従来のマップ方法で装用可能であったが, 場面装用に留まった症例や装用できていなかった症例では, 低域を裸耳聴力が 35dBHL に相当する周波数から電気刺激をするマップを好み, EAS の術前の補聴器装用の有無がマップの調整方法に影響していることが考えられた。EAS 症例に対する最適なマップを作成するためには, 術後の残存聴力だけでなく, 術前の補聴器装用状況を考慮することも必要である可能性が考えられた。
