Obsessive-compulsive disorder comorbid with rheumatoid arthritis: case report and review of literature

Introduction Obsessive-compulsive disorders are complex pathologies causing a major psychosocial handicap. However, their association with disabling somatic pathology such as rheumatoid arthritis makes management more difficult. Objectives To investigate through case analysis and review of literature the between obsessive-compulsive disorder (OCD) rheumatological disorders. Methods We reported woman long history obsessive compulsive who presented we conducted search on Pub-Med/MEDLINE following terms “obsessive-compulsive disorder”, “rheumatoid arthritis”, “association”, “inflammation”. Results Case presentation: A 62-years old had been followed at psychiatric consultation for 20 years OCD. She stabilised clomipramin dose 100 mg per day until 2012 since then she lost to follow-up notion poor compliance treatment. re-consulted in August 2022 worsening symptoms phobic obsessions delusional beliefs, verification compulsions, sleep multiple complaints including diffuse arthralgia chronic evolving 6 months. The patient was put risperidone 2mg anxiolytic without improvement. referred rheumatology where diagnosis very active retained, hence 20mg prednisolone methotrexate. evolution marked by accentuation compulsions appearance depressive elements, introduction fluoxetine 40 day, increase 4 an improvement symptoms.
