چکیده

Abstract Study question What are predictors of successful sperm retrieval (SRR) with microdissection testicular extraction (mTESE) in men non-obstructive azoospermia (NOA) according to different etiologies? Summary answer Predictors for SRR were patients various etiologies and effects age on idiopathic NOA KS opposite. is known already due spermatogenic dysfunction, accounting about 60 percent the total cases, may have opportunities obtain by mTESE overall 50%. There some predictive factors at including age, testis volume, serum follicle stimulating hormone (FSH), testosterone (T), inhibin B, anti-Mullerian histopathology, while remains underappreciated, especially accordance type etiology. Most notably, there controversies relationship between SSR surgery. design, size, duration This retrospective study involved 3104 treated first Reproductive Medical Centre Peking University Third Hospital from March 2012 December 2022. Participants/materials, setting, methods classified into seven groups 1530 males (iNOA), 763 Klinefelter syndrome (KS), 345 microdeletion AZFc, 177 cases history cryptorchidism, 131 a mumps orchitis, 89 cryptozoospermia 69 other causes. The end-point was presence one or more sperm. Multi-variable logistic regression used analyze outcome. Main results role chance Overall 43.46%, highest 89.31% orchitis lowest 29.35% iNOA patients. For negative T (0.805 [0.654, 0.991] p = 0.041). patients, shorter infertility (0.837 [0.704, 0.995], 0.044). Males bigger testes had likelihood positive (1.077 [1.005; 1.153], 0.035) cryptorchidism. BMI an independent factor AZFc-deleted (1.084 [1.013, 1.161], 0.020). lower male (0.938 [0.898, 0.979], 0.004) (1.214[1.099, 1.342], p<0.001) SRR. Lower levels FSH (0.984[0.970, 0.998] 0.025) higher age(1.079[1.050, 1.110], So, optimum range undergoing should be 30 35 years old 29.13% when considering low aged<30 (23.96%) effect female ICSI Limitations, reasons caution Surgeon performing maybe another predictor which potential selection bias. Inhibin B Anti-Müllerian weren’t included limited information our database. We couldn’t verify conclusion population multicenter despite current large sample size so far. Wider implications findings Our provide valuable who want counsel surgeons their treatments help perform shared decision-making optimal therapy methods, Trial registration number not applicable

Factors influencing sperm retrieval following testicular sperm extraction in nonobstructive azoospermia patients

OBJECTIVE Azoospermia owing to testicular disorders is the most severe manifestation of male infertility. The main concern for patients with nonobstructive azoospermia (NOA) is the probability of successful sperm retrieval following testicular sperm extraction (TESE). Therefore, the goal of this study was to determine predictive factors correlated with sperm retrieval. METHODS We assessed the...

Predictive Factors of Successful Microdissection Testicular Sperm Extraction in Patients with Presumed Sertoli Cell-Only Syndrome

Background To evaluate predictive factors of successful microdissection-testicular sperm extraction (MD-TESE) in patients with presumed Sertoli cell-only syndrome (SCOS). MaterialsAndMethods In this retrospective analysis, 874 men with non-obstructive azoospermia (NOA), among whom 148 individuals with diagnosis of SCOS in prior biopsy, underwent MD-TESE at Department of Andrology, Royan Institu...

Role of optimizing testosterone before microdissection testicular sperm extraction in men with nonobstructive azoospermia.

PURPOSE Although optimizing endogenous testosterone production before testicular sperm extraction is commonly practiced, whether improved preoperative testosterone levels enhance sperm retrieval remains unclear. We evaluated the influence of preoperative medical therapy in men with nonobstructive azoospermia before microdissection testicular sperm extraction. MATERIALS AND METHODS A total of ...
