Nonsurgical Repair of the Ascending Aorta: Why Less Is More

چکیده

Objective: Advanced endovascular options for acute and chronic pathology of the ascending aorta are emerging; however, several problems with stent grafts placed in have been identified patients unsuitable surgical repair, such as migration erosion at interface. Method: Among six cases analysed this report, three were treated a graft to manage dissection proximal aorta; dimensions those varied between 34 45 mm diameter, from 77 100 length. Three patients, matched by age, sex their nature pathology, subjected focal closure single communicating entry use an occluding device (Amplatzer ASD PFO occluders 14 18 disc diameter) similar Charlson comorbidity score. Results: Both conceptually different nonsurgical management strategies technically feasible; grafts, early or delayed full re-dissection was documented contrast complete seal, induced remodelling long-term survival after successful placing coils occluder devices. Moreover, aortic root motion not impaired occlusion communication occluder, while free impeded placement. Conclusions: The intriguing observation our small series that portends risk either (post-procedural) tissues landing site biological interface 12 16 months procedure, phenomenon seen devices up 5 years follow-up. Obviously, approach avoids wall result minimal interaction interface, diseased wall. Potential explanations may be related reduced placement aorta, whereas preserved occluder. causality however fully understood, besides stiffness radial force graft, other factors inflammatory reactions tissue local adhesions within chest also play role. With failing portend success, they still role temporizing solution elective conversion. Larger datasets registries needed further explore evolving field interventions aorta.

The knowledge paradox: why knowing more is knowing less

To provide an explanation of the evolution of scientific knowledge, I start from the assumption that knowledge is based on concepts, and propose that each concept about reality is affected by vagueness. This entails a paradox, which I term Knowledge Paradox (KP): i.e. we need concepts to acquire knowledge about the real world but each concept is a step away from reality. The KP provides a unify...

Results of surgical repair for dissection of the ascending aorta.

Between 1970 and 1986, 40 patients had surgical treatment for dissection of the ascending aorta at the London Chest Hospital. The overall hospital mortality was 27.5%. Preoperative renal impairment and age greater than or equal to 60 years were both associated with a significantly increased hospital mortality. In the long term one patient was lost to follow up. There have been two late deaths a...

Endovascular Repair of the Aortic Arch and Ascending Thoracic Aorta

Background The aortic arch is wide, compliant, and mobile. Its flow rate is high and its branches supply an organ, the brain, with no tolerance for ischemia. In this location, an endovascular stent graft is difficult to deliver, difficult to secure, and prone to fatigue fracture, graft erosion, or migration. The only advantage is the opportunity for downstream access to vital arterial branches,...

Less Is More, More the Merrier, or More From Less?

P hysicians deal with uncertainty all the time and chest pain in the emergency department (ED) is a typical example. Traditionally, coronary artery disease (CAD), pulmonary embolism (PE), and aortic dissection can present as chest pain, and the consequences of a missed diagnosis can be devastating, with the potential for rapid deterioration, and serious risk of morbidity and mortality. Moreover...
