Nifedipine disturbs fetal cardiac function during hypoxemia in a chronic sheep model at near term gestation

چکیده
BackgroundNifedipine is a widely used drug in pregnancies complicated by maternal hypertensive disorders that can be associated with placental insufficiency and fetal hypoxemia. The evidence regarding myocardial responses to nifedipine hypoxemia limited.ObjectiveWe hypothesized would not impair sheep cardiac function under hypoxemic environment. In particular, we investigated the effects of on ventricular functional parameters output.Study DesignA total 21 chronically instrumented at 122 134 gestational days (term, 145 days) were included this study. Fetal was evaluated measuring global longitudinal strain, indices describing systolic diastolic function, outputs using two-dimensional speckle tracking tissue spectral pulsed-wave Doppler echocardiography. carotid artery blood pressure gas values invasively monitored. After baseline data collection, induced hyperoxygenation. phase 9 fetuses received infusion, 12 saline infusion. Data collected 30 120 minutes after infusion started. oxygenation normalized, normoxemia collected, while continued.ResultsHypoxemia decreased mean arterial from 40 (8) mm Hg 35 (P<.007), left strain showed less deformation than (P=.001). Under hypoxemia, caused reduction right (P<.05), decrease isovolumic relaxation velocity its deceleration (P<.01) indicating dysfunction, drop output (P<.05). Nifedipine did alter or output. When restored, returned level.ConclusionIn fetus, impaired reduced detrimental abolished, when restored. Our findings suggest environment triggers function. limited. We A continued. Hypoxemia level.

Cardiac β-adrenergic receptor function in fetal sheep exposed to long-term high-altitude hypoxemia.
In this study, we hypothesized that a reduction in β-adrenergic receptor number or a decrease in functional coupling of the receptor to the adenylate cyclase system may be responsible for the blunted inotropic response to isoproterenol observed in fetal sheep exposed to high altitude (3,820 m) from 30 to 138-142 days gestation. We measured the contractile response to increasing doses of isoprot...

متن کاملChronic anemic hypoxemia increases plasma glucagon and hepatic PCK1 mRNA in late-gestation fetal sheep.
Hepatic glucose production (HGP) normally begins just prior to birth. Prolonged fetal hypoglycemia, intrauterine growth restriction, and acute hypoxemia produce an early activation of fetal HGP. To test the hypothesis that prolonged hypoxemia increases factors which regulate HGP, studies were performed in fetuses that were bled to anemic conditions (anemic: n = 11) for 8.9 ± 0.4 days and compar...
متن کاملFetal responses to long-term hypoxemia in sheep.
The effect of acute or short-term hypoxia on fetal cardiovascular hemodynamics has been well known; however, little is known about the effect of long-term hypoxemia. To determine the fetal hemodynamic responses to this stress we studied two groups of animals: 1) pregnant ewes (n = 20) at 110-115 days of gestation subjected to hypoxia for up to 28 days and 2) pregnant ewes (n = 4) that served as...
متن کاملEffects of long-term hypoxemia on pituitary-adrenal function in fetal sheep.
To test the hypothesis that long-term hypoxemia causes premature activation of the fetal pituitary-adrenal function, we embolized the fetal side of the placenta in pregnant sheep and examined the changes in concentrations of immunoreactive adrenocorticotropic hormone (irACTH), cortisol, and prostaglandin E2 (PGE2) in fetal plasma, and levels and localization of proopiomelanocortin (POMC) mRNA i...
