Multiclonal spread of Klebsiella pneumoniae across hospitals in Khartoum, Sudan

Multidrug-resistant (MDR) Klebsiella pneumoniae is increasing worldwide with poorly characterised epidemiology in many parts of the world, particularly Africa. This study aimed to investigate molecular K. pneumoniae, identify diversity sequence types (ST), and detect carbapenem resistance genes major regional hospitals Khartoum, Sudan. isolates (n = 117) were cultured from four April 2015 October 2016. The by sequencing 16S-23S rDNA internal transcribed spacer (ITS) region. Molecular was determined multilocus typing (MLST), analysed maximum likelihood phylogeny (PhyML). Antimicrobial susceptibility disk diffusion. Isolates phenotypically resistant screened for carbapenemase genes: blaNDM, blaOXA48, blaIMP, blaVIM blaGES PCR. ITS confirmed 117 as pneumoniae. MLST revealed 52 different STs grouped distinct clusters PhyML. All MDR, carbapenemase-producing (CP-KP) accounted 44/117 (37.6%) mostly harbouring blaNDM (28/44) blaOXA-48 (7/44), several multiple genes. MDR CP-KP widespread Khartoum hospitals, a diverse population clustering lineages. There an urgent need systematic epidemiological studies drug-resistant infections across all healthcare institutions Sudan inform local infection prevention control strategies.
