However, these selection criteria preclude the vast majority patients that undergo PCNL. The objective our study was compare complication stone free rates after aPCNL standard vs. extended METHODS: Retrospective review prospective data on all who underwent at one academic center from 2007-2018. Extended were defined as any: Age >75 years, BMI >30 kg/m2, ASA >2, bilateral stones, solitary kidney, staghorn calculi, burden >40 mm, multiple tracts, or pre-existing nephrostomy tubes/stents. Primary outcomes (Clavien-Dindo classification) (no fragments >/=3 mm). discharged with a ureteric stent no tube meeting discharge which included hemodynamic stability, fever, significant pain. RESULTS: We identified 118 92 (78%) met criteria. Mean 31 kg/m2 45% 3 higher. sum maximum diameter 24 mm. Multiple stones present 25%, 7%, 4%, tubes/stents 4%. There difference (12% 18%, p=0.56), Emergency department visit vs readmission (4% 5%, p=1) between respectively (Table 1). Of complications, 85% Clavien-Dindo grade 1. Stone not different (84%) (83%) (p=1). No variables associated complications univariate analysis. mm (OR 5.8, 95% CI 1.4-25.2, p=0.018) tracts (13.1, 1.1-154.7, p=0.041) residual fragments. CONCLUSIONS: Complication undergoing aPCNL. This supports safety efficacy using As COVID-19 pandemic continues strain hospital resources, offers solution deal growing backlog complex disease.
