Mitral Valve in Obstructive Hypertrophic Cardiomyopathy: Abnormalities, Management and Controversies

چکیده
Obstructive hypertrophic cardiomyopathy (obstructive HCM) is a hereditary disease characterized by septal hypertrophy and dynamic left ventricular outflow tract (LVOT) obstruction. Other than hypertrophy, mitral valve abnormalities are also quite common in patients with obstructive HCM, which may contribute to systolic anterior motion (SAM) of the LVOT Surgical myectomy standard treatment achieve anatomic correction but controversies remain on whether how procedures should be performed at same time. In this review, we first described HCM their surgical corrections, then explained based current clinical studies, finally made brief introduction our strategy results.

The Mitral Valve in Obstructive Hypertrophic Cardiomyopathy
Fro Su St. an Yo rel Ma Mitral valve abnormalities were not part of modern pathological and clinical descriptions of hypertrophic cardiomyopathy in the 1950s, which focused on left ventricular (LV) hypertrophy and myocyte fiber disarray. Although systolic anterior motion (SAM) of the mitral valve was discovered as the cause of LV outflow tract obstruction in the M-mode echocardiography era, in ...
متن کاملEchocardiographic assessment of mitral valve size in obstructive hypertrophic cardiomyopathy. Anatomic validation from mitral valve specimen.
BACKGROUND In patients with hypertrophic cardiomyopathy, obstruction to left ventricular outflow is produced by systolic anterior motion of the mitral valve. In many of these patients, the mitral leaflets are elongated and increased in overall size. Mitral valve size may be responsible, in part, for the presence and magnitude of the outflow gradient and the pattern of systolic anterior motion o...
متن کاملCombined anterior mitral valve leaflet retention plasty and septal myectomy in patients with hypertrophic obstructive cardiomyopathy.
OBJECTIVES Septal myectomy is the treatment of choice for patients with hypertrophic obstructive cardiomyopathy (HOCM) with significant left-ventricular outflow tract (LVOT) obstruction. In some HOCM patients, however, systolic anterior motion (SAM) of the anterior mitral leaflet significantly contributes to LVOT obstruction, resulting in mitral regurgitation and insufficient release of the obs...
متن کاملTransaortic mitral valve secondary chordal cutting in patients with obstructive hypertrophic cardiomyopathy and mild septal hypertrophy.
A proportion of patients with obstructive hypertrophic cardiomyopathy (HCM) and severe heart failure symptoms have only mild septal hypertrophy (1). In such patients, mitral valve (MV) abnormalities play an important role in systolic MV leaflets displacement into the left ventricular LV outflow tract and blood flow obstruction (1-3). Therefore, conventional septal myectomy alone may not be suff...

Background. The morphological determinants of mitral valve systolic anterior motion (SAM) and obstruction to left ventricular outflow in patients within the broad clinical spectrum of hypertrophic cardiomyopathy (HCM) are not completely understood, particularl the contribution of mitral leaflet length and size. Methods and Results. To clarify this issue, mitral valve specimens from 43 patients ...
