Supracondylar humerus fractures are common injuries encountered by orthopaedic surgeons.  Closed reduction and percutaneous pin fixation is classically considered the standard of care treatment for these injuries, as this method minimizes trauma to soft tissues, associated with few complications, have a high rate success.  However, loss fixation, when occurring, can lead devastating cosmetic, functional, neurological complications.  We present case study displaced supracondylar fracture initially treated three lateral divergent pins that lost underwent subsequent surgical revision. examine potential reasons keys successful revision.  &nbsp
