Long-term Control of Nasal Tip Position: Quantitative Assessment of Caudal Septal Extension Graft

Abstract Goals/Purpose Control of nasal tip position is critical in final rhinoplasty outcomes. Two frequent methods exerting control are columellar strut and caudal septal extension grafts. Past work has demonstrated that grafts better able to preserve rotation over time. However, there no quantitative data describing long term projection with grafts, which essential the aesthetic result. The purpose this study was analyze maintenance following graft. Methods/Technique A retrospective cohort adult patients undergoing open conducted. Open three-dimensional use graft were collated. Excluded closed rhinoplasty, cleft only, cases where deprojection or derotation implemented, less than 2 years follow up. All underwent osteotomies, projection, rotation. Three-dimensional photogrammetric evaluation performed preoperatively postoperatively (Vectra; Canfield Scientific, Parsippany, N.J.). Anthropometric points analyzed preoperatively, 1 week postoperative, 6 weeks at documented Outcome variables rotation, length. Nasal measured from piriform pronasale, length radix pronasale. Nasolabial angle defined using three (tip, subnasale, Cupid’s bow). Results compared statistically dependent t-test. Results/Complications total 20 included. Average up time 3.3 ± 1.3 (2.0 6.6 years). 15 primary 5 secondary rhinoplasty. Overall, 75 percent female, an average age 25.5 surgery.
