Abstract Background Laparoscopic cholecystectomy (LC) remains as the gold standard of surgical treatment for gallstone disease. Biliary duct injury (BDI) is an infrequent but serious complication LC. Strasberg's critical view a useful strategy to minimize risk BDI . However, BDIs could still happen. Variations right posterior hepatic (RPHD) are common. The RPHD challenging and literature on this matter scarce. Case summary A berrant drainage into gallbladder neck was unexpectedly identified in 43-year-old man during laparoscopic cholecystectomy. Intraoperative consultation with specialized Hepatobiliary surgeon accomplished anastomosis between jejunum Roux-en-Y reconstruction carried out. operation uneventful no long-term reported over 12-month follow-up period. Conclusion Aberrant implantation must be borne mind despite its low incidence. Previous studies reporting management In our case, hepaticojejunostomy by experienced HPB successful solve difficult case.
