Isoprenaline and quinidine to calm Brugada VF storm.
A 3-year-old man with an implanted cardioverter-defibrillator (ICD) for type 2 Brugada syndrome was admitted to coronary care unit with ventricular fibrillation electrical storm and consequent appropriate recurrent ICD shocks. The rhythm did not settle with conventional antiarrhythmics such as β-blockers and amiodarone. An isoprenaline infusion was set up with immediate stabilisation of rhythm....
متن کاملQuinidine: an "Endangered Species" Drug Appropriate for Management of Electrical Storm in Brugada Syndrome
Brugada syndrome is an inherited channelopathy associated with an increased risk of syncope and sudden cardiac death. In rare cases it can be manifested with electrical storm. We report two cases of Brugada syndrome that presented with electrical storm and were treated successfully with oral quinidine, an "endangered species" drug.
متن کاملSuccessful use of quinidine in treatment of electrical storm in Brugada syndrome.
We report an adolescent with a malignant form of Brugada syndrome who presented with 15 episodes of ventricular fibrillation (VF) over 10 days, shortly after implantation of an implantable cardioverter defibrillator. Oral quinidine bisulphate at a dose of 1000 mg/day successfully suppressed the electrical storm and recurrence of VF over 18-month follow-up. It also normalized the ST-segment elev...
متن کاملA practical guide to volatility forecasting through calm and storm
We present a volatility forecasting comparative study within the autoregressive conditional heteroskedasticity (ARCH) class of models. Our goal is to identify successful predictive models over multiple horizons and to investigate how predictive ability is influenced by choices for estimation window length, innovation distribution, and frequency of parameter reestimation. Test assets include a r...
متن کاملQuinidine for Brugada syndrome: Panacea or poison?
Brugada syndrome is associated with sudden cardiac death in young patients. Currently guidelines recommend the use of implantable cardioverter-defibrillator (ICD) in high-risk patients to prevent sudden cardiac death and adjunctive therapy with quinidine in those who have recurrent ventricular tachyarrhythmias to prevent electrical storm. Nevertheless, not all patients respond to quinidine, and...
