Heart transplantation is the standard of care treatment for end-stage heart failure. Therapeutic advances including enhanced immunosuppression and aggressive infectious prophylaxis have led to increased life-expectancy following transplantation; however, cardiac allograft vasculopathy (CAV) remains a leading cause morbidity mortality. Although coronary angiography current guideline-recommended diagnostic modality invasive CAV screening, it limited in its ability detect early and/or diffuse disease. Efforts improve outcomes transplant recipients with focused on developing tools greater sensitivity capture order better understand pathobiology implement slow disease progression sooner after transplant. The contemporary imaging armamentarium surveillance includes angiography, intravascular ultrasound, newer technologies optical coherence tomography near-infrared spectroscopy. present review outlines use data support these platforms arena highlights potential advantages limitations each modalities.
