چکیده

Abstract Introduction Oral anticoagulants are the standard treatment for prevention of stroke in patients with atrial fibrillation (AF). However, some still have despite anticoagulation or contraindications to anticoagulation. The left appendage occlusion (LAAO) is an option those patients. use intracardiac echocardiography (ICE) instead Transesophageal guiding LAAO procedures has increased, allowing reduce general anesthesia. Objectives aim this study describe data regarding safety and efficacy submitted ICE-guided LAAO. Methods In a tertiary center, were identified. Information baseline characteristics, procedure technical success, complications, hospitalization follow-up was noted retrospectively. registered bleeding rates compared predicted using CHA2DS2-VASc HAS-BLED scores, respectively. Results 45 underwent LAAO, mean age 75.5±9.6 years old, 71.1% male (detailed characteristics table 1). Permanent AF present 68.9% (n=31), average scores 4.0±1.4 3.6±1.1, respectively (predicting risk 4.0% per year major 8.7% year). indication previous 64.4% (n=29), high 24.4% (n=11) embolic events therapeutic 11.1% (n=5). devices implanted successfully 96% (n=43). It complication rate 8.8% (n=4), perforation 4.4% (n=2), device embolization (n=2, one them leading cardiac arrest death) no vascular complications occurred. No other procedure-related deaths duration after 5.7 days. All followed-up period 19.0 months. During that period, another death (2.2%) occurred non-cardiac cause re-hospitalization 26.7% (n=12), (n=5) causes. same 1 6 (13.3%) – yearly 1.4% 8.4%, markedly inferior by score slightly predicted. Conclusions This provides about mainly guided ICE cohort hemorrhagic risk. Funding Acknowledgement Type funding sources: None.

A Semi-Automated Algorithm for Segmentation of the Left Atrial Appendage Landing Zone: Application in Left Atrial Appendage Occlusion Procedures

Background: Mechanical occlusion of the Left atrial appendage (LAA) using a purpose-built device has emerged as an effective prophylactic treatment in patients with atrial fibrillation at risk of stroke and a contraindication for anticoagulation. A crucial step in procedural planning is the choice of the device size. This is currently based on the manual analysis of the “Device Landing Zone” fr...

Left Atrial Appendage Occlusion

Left atrial appendage (LAA) occlusion is a treatment strategy to prevent blood clot formation in atrial appendage. Although, LAA occlusion usually was done by catheter-based techniques, especially percutaneous trans-luminal mitral commissurotomy (PTMC), it can be done during closed and open mitral valve commissurotomy (CMVC, OMVC) and mitral valve replacement (MVR) too. Nowadays, PTMC is perfor...

The role of echocardiography in percutaneous left atrial appendage occlusion.

Percutaneous device closure of the left atrial appendage (LAA) has been introduced in the last decade as a minimally invasive alternative treatment to long-term anticoagulation to reduce the risk of thrombo-embolism in patients with atrial fibrillation. Echocardiography is an essential tool at all stages of the procedure. Pre-procedural echocardiography is used to screen suitable candidates and...

Catheter-based left atrial appendage occlusion procedure: role of echocardiography.

Atrial fibrillation is a common, clinically significant arrhythmic disorder that results in increased risk of morbidity and mortality in affected patients. Atrial fibrillation is more prevalent among men compared with women and the risk for developing atrial fibrillation increases with advancing age. Ischaemic stroke is the most common clinical manifestation of embolic events from atrial fibril...
