TOPIC: Lung Cancer TYPE: Medical Student/Resident Case Reports INTRODUCTION: Large-cell neuroendocrine carcinoma (LCNEC) is a rare form of pulmonary tumors with features small cell lung and non-small cancer. Due to its rarity, there little known about presentation prognosis. We review 2 cases LCNEC different outcomes which will help contribute the current literature LCNEC. CASE PRESENTATION: Patient A was 76-year-old male who presented Emergency Department chief complain dyspnea shortness breath. had history COPD 4-5L home oxygen, hypertension, right mass. smoker 100 pack-year history. He reported recent unintentional weight loss 40 pounds. CT Chest demonstrated chest mass measuring 11.9cm. B 43-year-old no prior smoking ED complaints myalgias, sore throat, fevers, also began having generalized weakness, 20lbs, hoarseness increased over past month. CTA large multi-spatial mediastinal confluent adenopathy extending left hilum. Both patients underwent endobronchial ultrasound-guided biopsy bronchioalveolar lavage that lung. diagnostic studies 4 month after detected following day detection. experiences an extended stay in medical intensive care unit ultimately admitted hospice. expired 24 hours admission systemic chemotherapy radiation oncologist currently improving. DISCUSSION: Further are aimed towards finding similarities symptoms or laboratory values order identify this aggressive earlier course. Our case reports highlight nature It reveals advanced age, comorbidities delayed diagnosis all feed into poor prognosis for CONCLUSIONS: cancer warrants expediated work-up early treatment.
