Infective endocarditis caused by Klebsiella pneumoniae in a patient with non ST elevation myocardial infarction

A rare clinical case of native aortic valve infective endocarditis (IE) caused by Klebsiella pneumoniae in a 56-year old man without known risk factors predisposing to the development IE is presented. Diagnosis this patient was challenge due lack recent interventions that could be considered as source bacteremia, scarce manifestation and absence typical complications. Aortic vegetation detected transesophageal echocardiography. K. isolate susceptible all antibiotics tested. Antibacterial therapy (cefepime 6 g/day IV for 2 weeks hospital followed ceftriaxone 4 IM cefixime 400 mg/day PO, total an outpatient) resulted complete resolution signs symptoms, laboratory abnormalities well size decrease. Surgical treatment not required patient.
