BackgroundHome visits (HVs) may be warranted for many reasons (e.g., uncontrolled disease states, suspected psychosocial issues, frequent hospitalizations, poor health literacy). Patients who frequently visit the emergency department (ED) are high-risk individuals, oftentimes exhibiting health-related barriers and medication-related problems (MRPs).ObjectivesThis study seeks to answer whether HVs ED users conducted by a pharmacist with pharmacy students will detect more MRPs compared office (OVs) enhance patient perception of HV services.MethodsPatients visited at least twice over 12-month period were included in retrospective chart review. Eligible patients randomized into an group or OV group. identify resolve MRPs, whereas arm brought their medications completed pre- postvisit survey about experiences.ResultsEighteen participated study: 10 8 arm.
