چکیده

OBJECTIVE We examined diabetes as a cardiovascular disease (CVD) risk equivalent based on severity and other CVD factors. RESEARCH DESIGN AND METHODS pooled four U.S. cohorts (Atherosclerosis Risk in Communities [ARIC], Jackson Heart Study [JHS], Multi-Ethnic of Atherosclerosis [MESA], Framingham Offspring Cohort [FHS-Offspring]) classified subjects into groups by baseline diabetes/CVD status (positive or negative). risks the mellitus (DM)+/CVD? group versus DM?/CVD+ were subgroups developed an algorithm to identify with comparing relative being DM+/CVD? DM?/CVD+. RESULTS The cohort included 27,730 (mean age 58.5 years, 44.6% male). rates per 1,000 person-years 16.5, 33.4, 43.2, 71.4 among those DM?/CVD?, DM+/CVD?, DM?/CVD+, DM+/CVD+, respectively. Compared similar higher for HbA1c ? 7%, duration ?10 medication use, while less severe had lower risks. Hazard ratios (95% CI) vs. 0.96 CI 0.86–1.07), 0.97 (0.88–1.07), (0.82–1.13), 1.18 (0.98–1.41), 0.93 (0.85–1.02), 1.00 (0.89–1.13) women White race, <55 triglycerides ?2.26 mmol/L, hs-CRP ?2 mg/L, estimated glomerular filtration rate <60 mL/min/1.73 m2, In group, 19.1% score but observed risk. CONCLUSIONS Diabetes is one-fifth CVD-free adults living diabetes. High HbA1c, long duration, use predictors equivalence. women, white people, younger reduced kidney function.
