Femoral shaft fractures are among the most prevalent of lower extremities in children. Although many approaches and options available for their treatment, appropriate treatment option school going children is still debatable. In this study 34 with femoral were recruited between Jan. 2018 Jan 2019 at Nangarhar university public health Hospital. which divided into two groups: (Group-A) immediate reduction hip Spica consists 22 (Group -B) consist skin traction 2-3 weeks precede casting 12 we studied gender distribution, fracture side, site type fracture, also note complications include; initial shortening, axial, sagittal rotational alignment hospital stay. All patients under observation a period 3 months after removal cast. The found that boys more affected than girls, middle third was common (58.92%), spiral (43.18%), as well side way observed other kinds both modalities; Spica, four (18.18%) three cases (25%).Conclusions: There minimal difference early delayed terms functional result. However, greatly reduced hospitalization time costs while producing equivalent results use.
