The histopathological changes in Budd–Chiari syndrome (BCS) overlap with those of sinusoidal obstruction (SOS) and cardiac or pericardiac disorders resulting right failure. These conditions, however, are different on both clinical pathological grounds need to be differentiated from BCS. This review is centred the three main aspects BCS diagnostic liver histopathology: (1) general histopathology BCS; (2) implications for biopsy interpretation; (3) allograft. histological features form a complex spectrum which shaped differently each individual case according topographical distribution chronological evolution obliterative insult, its upstream effect hepatic vascularisation consequent parenchymal injury, scarring remodelling.
