Healthcare Professionals’ Perspectives on the Outgoing Geriatric Team: A Qualitative Explorative Study

چکیده

The need for communication and collaboration increases when an older patient is discharged from the hospital, as transition of care complex patients living with multiple concurrent diseases. An intervention: outgoing geriatric team was developed initiated to address these patients’ needs. aimed collaborate healthcare professionals at a skilled nursing facility. This study explored how intervention experienced by both employed qualitative explorative design using semi-structured interviews. Fourteen participated in Data were analyzed Braun Clark’s thematic analysis.
