Gastric bezoar in a patient hospitalized in an eating disorder unit. Case report

Introduction It is well known that eating disorders are related to comorbidity. At least, half of these patients have other mental and, in addition it, the presence physical comorbidity (cardiovascular, kidney, nervous system, digestive tract, metabolic or endocrine disorders) comes with a decline life expectancy. Objectives Description patient diagnosis anorexia nervosa (AN) who developed gastric bezoar during hospitalization. Methods Case treated specific Eating Disorder Unit Third-Level Hospital. Results 26 years old woman AN hospitalized General Psychiatric BMI 11,78 kg/m2. Nasogastric tube was necessary after 1 month progressive weight recovery (BMI 13,84 kg/m2), transferred order follow psychological therapy. No incidence exploration clinical analyses happened this apart from pancytopenia due malnutrition. However, 8 days after, nausea and had 3 vomit episodes, constant abdominal pain at hipogastrium (moderate intensity), dizziness, instability constipation. The refused possibility pregnancy. exam showed bowel sounds augmented but no mass peritoneal irritation appeared. Blood test results were normal. Abdominal X-Ray dilatation small faeces sign, which suggested bezoar. treatment dissolution by Coca-Cola, solving symptoms completely. having eaten hair any kind object indigestible material admitted be following strict vegan diet. Finally, an endoscopy done, diagnosed phytobezoar. Conclusions Based on literature, bezoars rare AN, being phytobezoars most common between types bezoars. Nevertheless, there some risk factors, such as delayed emptying, dehydration or, case phytobezoar, ingestion food containing high amount cellulose, hemi-cellulose, lignin, tannins (celery, pumpkin, grape skins, prunes, raisins particular, persimmons). Some caused phytobezoar can similar those (abdominal pain, intestinal obstruction, poor appetite, vomiting, malnutrition, loss). Therefore, could underdiagnosed even undiagnosed disease group patients. Taking into account reduce time until treatment, decreasing risks associated. Disclosure Interest None Declared
