Free tissue transfer for skull base reconstruction - a review

چکیده
Evolutions in skull base surgery and reconstructive technique have given surgeons the confidence to resect repair increasingly advanced pathologies. Free tissue transfer (FTT) provides a versatile option capable of addressing numerous simultaneous goals. This review highlights some nuances, challenges, considerations performing FTT for reconstruction anterior, central lateral base. combines expert opinion senior authors with those field at large as queried through PubMed searches regarding FTT. Reconstructive goals include separation intracranial from extracranial cavities, obliteration dead space, protection vascular neural structures. Atypical pedicle management is commonly needed, especially endonasal resection. Virtual surgical planning may be beneficial complex bony reconstruction. Familiarity common complications such cerebrospinal fluid leak, nasocutaneous fistula, inferior flap displacement, well associations their development, can help plan minimize morbidity.

Skull base reconstruction.
Various surgical techniques for extirpation of anterior skull base tumors have been established over the past few years. Tumors that are located in this anatomical area may invade both soft and hard tissues of the skull base. In such cases, an en bloc tumor resection may create extensive skull base defects and produce a free conduit between the paranasal sinuses and the intracranial space. Foll...
متن کاملLocoregional and free flap reconstruction of the lateral skull base.
Lateral temporal bone reconstruction after ablative surgery for malignancy, chronic infection, osteoradionecrosis, or trauma presents a challenge for the reconstructive surgeon. This complexity is due to the 3D nature of the region, potential dural exposure, and the possible need for external surface repair. Successful reconstruction therefore requires achieving separation of the dura, oblitera...
متن کاملOutcomes following Microvascular Free Tissue Transfer in Reconstructing Skull Base Defects.
Objective Successful resection of complex tumors involving the skull base (SB) depends on the ability to reconstruct the resulting defects. The objective of this study was to assess the outcomes of patients undergoing reconstruction after resection of SB tumors with free flaps. Methods From 1995 to 2010 a retrospective review of cases was undertaken. Demographics, histology, surgical management...
متن کاملReconstruction in skull base surgery: Review of current concepts
Surgical resection plays a main role in the management of complex tumors involving the skull base. Advances in reconstructive surgery has made these resections to be carried out with less morbidity and better outcome in terms of disease control and functional rehabilitation. The reconstruction methods range from utilizing local galeal tissue to distant tissues transferred as free ß aps. The rec...
