Extensive Extradural Lipomatosis / Lipomatose Extradural Extensiva

A male patient presents with urinary incontinence worsening in the last year, shock sensation, symmetrical weakness and involuntary spasms lower limbs. He currently required double support to ambulate had no sphincter control. Neurological examination demonstrated limb hypertonia spastic paraplegia severe impairment of deep sensation painful muscle spasms. The normal body mass index laboratory tests excluded endocrinopathies. MRI showed myelopathy caused by hyperintense tissue suppressed T1 signal epidural space, representing fatty extensive hyperintensity upper portion thoracic spinal cord. Axial T2-weighted imaging confirmed space between T3 T7 levels, promoting markedly narrowed dural sac cord compression. case was then compatible idiopathic lipomatosis surgery indicated. did not agree lost follow-up.
