Evaluation of Adjusted Calcium Levels in Patients of Chronic Renal Failure with Hypo-Albuminemia

Objective: To compare adjusted calcium levels with ionised Calcium in chronic renal failure patients hypoalbuminemia for the correct assessment of their status.
 Study Design: Cross-sectional study.
 Place and Duration Study: Department Chemical Pathology Endocrinology, Armed Forces Institute Pathology, Rawalpindi Pakistan, from Mar 2020 to Jan 2021.
 Methodology: A total 304 individuals aged 18 90 years, either gender were included study. Participants differentiated into stages Chronic based on estimated glomerular filtration rate calculated through Kidney Disease Epidemiology Collaboration equation. Serum Calcium, urea creatinine analysed ADVIA 1800R Clinical Chemistry Auto analyser. In addition, was Cobas b221 blood gas electrolyte analyser.
 Results: Mean age study participants 56.3±16.03 years. 130(42.5%) had stage-5 while 102(33.3%) stage 4 kidney disease. Based concentration, 37.3% hypocalcaemia, 57.2% normocalcaemia, 4.9% hypercalcaemia. 57.5% albumin concentration between 20-29g/L. Cohen’s Kappa statistical analysis showed be a poor predictor status hypoalbuminemia.
 Conclusion: Adjusted using Modified Payne formula, overestimates when compared unadjusted hence misclassifying true hypocalcaemic as normocalcaemic.Keywords: calcium, failure, Estimated rate, Hypoalbuminemia, Ionised calcium.
