Alzheimer disease and other dementias present unique practical challenges for patients, their families, clinicians, health systems. These reflect not only the growing public effect of dementia in an aging global population, but also more specific ethical complexities including early loss patients' capacity to make decisions regarding own care, stigma often associated with a diagnosis, difficulty balancing concern welfare respect remaining independence, on physical, emotional, financial well-being family caregivers. Caring patients requires respecting patient autonomy while acknowledging progressively diminishing decisional continuing provide care accordance core principles (beneficence, justice, nonmaleficence). Whereas these remain unchanged, neurologists must reconsider how apply them given changes across multiple domains our understanding disease, clinical legal tools addressing manifestations illness, expanding awareness crucial role caregivers providing maintaining quality life, societal conceptions individuals' personal expectations aging. This revision American Academy Neurology's 1996 position statement summarizes considerations that arise caring dementia; although it addresses such influence management, is practice guideline.
