Abstract Aim To present a rare incidental synchronous triple tumours - colonic adenocarcinoma, endometroid ovarian adenocarcinoma and benign Brenner tumour in patient admitted with acute gallstone pancreatitis. Methods A 75-year-old female presented epigastric pain. Blood-tests USS abdomen confirmed gallstones She was treated conservatively. also showed pelvic mass which further characterised by CT MR scans. These scans heterogeneous lobulated the left adnexa. scan picked-up transverse colon, as cancer colonoscopy. offered therapeutic resection after discussion Colorectal Gynaecology MDT. Intra-operatively, invading into proximal ileum. Laprascopic surgery concluded extended right hemicolectomy, small bowel resection-anastomosis, omentectomy, total abdominal hysterectomy bilateral salpingo-oophorectomy. Results The had gradual recovery without any complications. Histopathology T4N2 poorly differentiated of mass, FIGO grade 1 stage 1c endometrioid ovary brenner ovary. Post-operative MDT did not offer adjuvant-therapy due to slow fraility. So far, two-years follow-up show recurrence. Conclusion This report adds limited literature tumours, including accounting for 5% an incidence 4 -7%. Multi-disciplinary approach combined can achieve desirable outcome such complex cases. It is crucial identify primary status it will guide adjuvant treatment.
