چکیده

Background: Expanded-endoscopic endonasal approaches (eEEA) to surgical targets around the internal carotid artery (ICA) remain inspiring and necessitate a special learning curve. Methods: Six formalin-fixed heads were dissected study 360° accessibility manoeuvrability ICAs from distal dural ring (DDR) foramen lacerum (FL) on each side (n=12) via eEEA. The ICA course was divided into three segments: in cavernous sinus (CS-ICA), paraclival-ICA lacerum-ICA. Following total exposure of ICA, three-dimensional (3D= medial/lateral, superficial/deep above/below) anatomical (n= 35) evaluated. data collected analyzed. Results: Distinctively, we included 35 (pertinent CS-ICA, lacerum-ICA 16, 10, 9, respectively) every DDR FL. Although reasonable visual validation possible for all targets, micro-dissections safely achievable 19 (54.3%) through full or limited freedom 73.7% 26.3%, respectively. In remaining 45.7%, abandoned due hazardous manipulation. most unreachable (55.6%) with extremely restricted maneuverability (75%). Though, favorable (60%) (100%) paraclival-ICA. Conclusion: This delivers distinctive view appreciate degree complexity invasiveness relation Illustrating these details skull base surgeons can lead them tailor their approach. It prospective feedback our operative theatre.

Endoscopic Endonasal Repair of Internal Carotid Artery Injury during Endoscopic Endonasal Surgery

Background  Injury to the cavernous portion of the internal carotid artery (ICA) during endoscopic skull base surgery is a well-recognized rare complication that can be associated with high rates of morbidity and mortality. Many techniques have been suggested to manage ICA injury with varying degrees of success. Objectives  We provide a detailed technical description of an operative technique f...

Analysis of the petrous portion of the internal carotid artery: landmarks for an endoscopic endonasal approach.

OBJECTIVES/HYPOTHESIS While there are many benefits to the endoscopic endonasal approach to the infratemporal fossa, involvement of the petrous portion of the internal carotid artery (ICA) poses a unique challenge. The endoscopic endonasal approach requires establishing the relationship of the petrous ICA to anatomical landmarks to guide the surgeon. This study evaluates the relationship of pet...

Internal carotid artery injury in endoscopic endonasal surgery: A systematic review.

OBJECTIVES/HYPOTHESIS Internal carotid artery (ICA) injury during endoscopic endonasal surgery (EES) is a known and feared complication of paranasal sinus and skull base procedures. These ICA injuries can result in stroke, cranial nerve palsies, and death. This review examines the setting of injury along with the treatment approaches, and patient outcomes. STUDY DESIGN Systematic review using...
