Since the inception of jack-up rig operations in early 1950s, series hazardous accidents has been witnessed, and losses recorded. These events are approximated to occur at least twice a year. With this statistics, harsh offshore environment demands jack-ups premium quality. Designing self-elevating (jack-up) structure is one that must be founded on well-proven principles, industry-accepted reliability techniques criteria. Construction also follow suitable classification society’s guidelines for different aspects phases jack-up.Before designing constructing which can optimally operate within environment, identifying hazards associated with how mitigate or manage them essential. The Formal Safety Assessment (FSA) methodology employed maritime industry identify check these marine hazards, design ways bring under control cost effective manner. five steps FSA hazard identification, risk assessment, option, benefit analysis/assessment, decision making steps. In paper, was introduced its operation principle explained from transit mode offshore. units were identified using Preliminary Hazard Analysis (PHA). Capsizing as critical considered analyzed Fault Tree (FTA). Consequently, Fishbone Diagram used analyze, state root cause capsizing structures through factors sampled incident cases, recommend measures curb special problem. Some recommended bolster existing guidance. Recommendations improvements eliminate reduce construction, before in-service made.
