چکیده
Abstract Background The use of catheter-based thrombolysis in comparison to systemic thrombolytics has emerged as a potentially lifesaving therapy for patients with sub massive or pulmonary embolism (PE). addition ultrasound waves accelerate lytic dispersion assisted (USAT) been proposed improve outcomes compared standard catheter-directed (SCDT). These two modalities have small studies, but larger population data on the these therapies is still lacking. Purpose To assess efficacy and safety USAT versus SCDT submassive embolism. Methods A review electronic databases (PubMed, Scopus, Embase, Cochrane) was performed using keywords USAT, PE studies were included if between types therapies. Efficacy that evaluated reduction right ventricle/left ventricle ratio (RV/LV ratio), artery systolic pressure (PASP) Miller score. Safety major bleeding, mortality length ICU stay. Standardized mean differences (SMD) 95% confidence intervals (CI) calculated continuous variables. Risk (RR) CI reported dichotomous variables random effects model comprehensive meta-analysis RevMan 5.4.1 softwares. Results Six total 381 (USAT n=204; n=177) included. No statistical difference over noted degree RV/LV (SMD: −0.507; CI: −1.386–0.373; p>0.05; I2=92%), PASP 0.037; −0.404–0.478; I2=59%), score 0.303; −0.481– 1.087; I2=82%). also not statistically different similar rates bleeding (RR: 1.44; 0.54–3.85; I2=4%), 1.46; 0.35–6.05; I2=0%) stay −0.01; −0.29–0.27; I2=31%) both treatment groups. Conclusion Our suggest despite technological advancement there no additional benefit terms safety. Further are warranted procedures investigating financial clinical real world practice. Ultrasound-assisted catheter-directed thrombolysis for pulmonary embolism
We have read through the case report article with great interest, entitled “Combined catheter thrombus fragmentation and percutaneous thrombectomy in a patient with massive pulmonary emboli and acute cerebral infarct,” by Uğurlu et al. (1) and published in Anatol J Cardiol 2015; 15: 69-74. For the last two years, ultrasound-assisted catheter-directed thrombolytic (USAT) has been used as an alte...
متن کاملSystematic review and meta-analysis for thrombolysis treatment in patients with acute submassive pulmonary embolism
PURPOSE The aim of this systematic review was to evaluate the efficacy and safety of thrombolytic treatment in patients with submassive pulmonary embolism (PE). METHODS An electronic search was carried out based on the databases from MEDLINE, Embase, Science Citation Index (SCI), and the Cochrane Library. We included prospective, randomized, and clinical trials in thrombolysis with heparin al...
متن کاملEffectiveness and cost-effectiveness of thrombolysis in submassive pulmonary embolism.
BACKGROUND Thrombolytic therapy is controversial in patients with submassive pulmonary embolism. METHODS We performed a cost-effectiveness analysis to compare health effects and costs of treatment with alteplase plus heparin sodium vs heparin alone in hemodynamically stable patients with pulmonary embolism and right ventricular dysfunction by developing a Markov model and using data from clin...
متن کاملLocal Thrombolysis for Acute Massive Pulmonary Embolism using a Pulse-Infusion-Thrombolysis Catheter
Acute massive pulmonary embolism (PE) is a common life-threatening condition requiring emergent and suitable treatment. The aim of this study is to assess the efficacy and safety of local thrombolysis with a pulse-infusion-thrombolysis (PIT) catheter in the management of acute massive PE. Thirty-nine patients with PE were treated with catheter directed intervention (CDI). CDI involves suction e...
متن کاملUltrasound-assisted thrombolysis for acute pulmonary embolism: a systematic review.
Pulmonary embolism remains a common and potentially life-threatening disease. For patients with intermediate- and high-risk pulmonary embolism, catheter-based revascularization therapy has emerged as potential alternative to systemic thrombolysis or surgical embolectomy. Ultrasound-assisted catheter-directed thrombolysis is a contemporary catheter-based technique and is the focus of the present...
