Effect of the vancomycin minimum inhibitory concentration on clinical outcomes in patients with methicillin-susceptible Staphylococcus aureus bacteraemia: a systematic review and meta-analysis

چکیده
Objective The use of the vancomycin minimum inhibitory concentration (MIC) as a prognostic predictor in patients with methicillin-susceptible Staphylococcus aureus (MSSA) has been debated last decade. We performed systematic review and meta-analysis to investigate whether an elevated MIC is associated worse prognosis for MSSA bacteraemia. Design Systematic meta-analysis. Data sources PubMed, Embase Cochrane Library were searched from inception December 2019. Eligibility criteria Randomised controlled trials or observational studies considered eligible if they provided clinical outcomes bacteraemia, stratified by MIC. synthesis Primary outcome was mortality. Secondary included septic thrombophlebitis, persistent bacteraemia complicated Pooled ORs 95% CIs calculated. Subgroup analyses susceptibility testing method. Results Fifteen included. Bacteraemia due isolates high MICs higher mortality than low (OR 1.44; CI 1.12 1.84; I 2 =40.3%). Additionally, significantly greater thrombophlebitis 3.16; 1.11 9.00; =58.6%) trend towards more 1.79; 0.97 3.31; =0%) observed MICs. Differences not significant. Similar findings obtained subgroup using Etest. However, significant differences between detected broth microdilution. Conclusion available data suggest association Etest adverse Future should validate these explore potential mechanisms. PROSPERO registration number CRD42018090547.

High vancomycin minimum inhibitory concentration and clinical outcomes in adults with methicillin-resistant Staphylococcus aureus infections: a meta-analysis.
BACKGROUND Patients with methicillin-resistant Staphylococcus aureus (MRSA) infections caused by isolates with a high but 'susceptible' minimum inhibitory concentration (MIC) to vancomycin may suffer poor outcomes. The aim of this study was to determine the association of high compared to low vancomycin MICs and clinical outcomes (treatment failure and mortality) in patients with MRSA infection...

During the last years, antimicrobial resistance has become one of the greatest challenges in clinical settings. Staphylococcus aureus and Enterococcus are important nosocomial pathogens worldwide. In different corners of the world, methicillin-resistant Staphylococcus aureus (MRSA) and vancomycin-resistant enterococci (VRE) have been isolated from various sources including different foods of an...

Correspondence Role of High Vancomycin Minimum Inhibitory Concentration in the Outcome of Methicillin-Susceptible Staphylococcus aureus Bacteremia To the Editor—We read with great interest the article by Holmes et al [1] and the editorial commentary published in the same issue [2] regarding the novel topic of the potential role of high minimum inhibitory concentration (MIC) levels of vancomycin...
متن کاملThe clinical significance of vancomycin minimum inhibitory concentration in Staphylococcus aureus infections: a systematic review and meta-analysis.
BACKGROUND Emerging data suggest that vancomycin may be less effective against serious methicillin-resistant Staphylococcus aureus (MRSA) infections with minimum inhibitory concentration (MIC) values at the higher end of the susceptibility range. The purpose of this review is to examine the strength of these associations. METHODS All relevant studies pertaining to treatment outcomes or mortal...
متن کاملVancomycin minimum inhibitory concentration in methicillin-susceptible Staphylococcus aureus and mortality: more than handwaving?
TO THE EDITOR—We read with great interest the study by Cervera and colleagues on the effect of vancomycin minimum inhibitory concentration (van-MIC) on clinical outcome in 93 patients with leftsided infective endocarditis due to methicillin-susceptible Staphylococcus aureus (MSSA) [1]. However, various questions remain unanswered. The 3-fold higher mortality in the group of patients with a van-...
