چکیده
Aims To study loop diuretic response and effect of omission in ambulatory heart failure (HF) patients on chronic low-dose diuretics. Methods results Urine collections were performed two consecutive days 40 HF with 40–80 mg furosemide (day 1 diuretic; day 2 without diuretic). Three phases collected each day: (i) first 6 h; (ii) rest the day; (iii) night. On intake, total natriuresis was 125.9 (86.9–155.0) mmol/24 h urine output 1650 (1380–2025) mL/24 h. There a clear 9.4 (6.7–15.9) mmol/h 117 (83–167) mL/h during h, followed by significant drop [2.6 (1.8–4.8) 55 (33–71) mL/h] night [2.2 (1.6–3.5) 44 (34–73) mL/h]. 2, after omission, remained similarly low entire day, resulting 50% reduction [55.1 (33.5–77.7) P < 0.001] 31% [1035 (875–1425) compared intake. Conclusion Patients treatment still have phase, while leads to output, arguing against routine cessation

متن کاملLoop diuretic efficiency: a metric of diuretic responsiveness with prognostic importance in acute decompensated heart failure.
BACKGROUND Rather than the absolute dose of diuretic or urine output, the primary signal of interest when evaluating diuretic responsiveness is the efficiency with which the kidneys can produce urine after a given dose of diuretic. As a result, we hypothesized that a metric of diuretic efficiency (DE) would capture distinct prognostic information beyond that of raw fluid output or diuretic dose...
متن کاملRapid and Highly Accurate Prediction of Poor Loop Diuretic Natriuretic Response in Patients With Heart Failure.
BACKGROUND Removal of excess sodium and fluid is a primary therapeutic objective in acute decompensated heart failure and commonly monitored with fluid balance and weight loss. However, these parameters are frequently inaccurate or not collected and require a delay of several hours after diuretic administration before they are available. Accessible tools for rapid and accurate prediction of diu...

The fundamental role of sodium in maintenance of fluid homeostasis and progression of heart failure (HF) has been a central paradigm in medicine for more than a century. As a result, a core treatment principal in HF management has been the avoidance of dietary sodium. However, accumulating evidence ranging from the basic sciences to epidemiological studies has challenged this paradigm, suggesti...
متن کاملCombined aquaretic and diuretic therapy in acute heart failure
INTRODUCTION Acute heart failure (AHF) is a leading cause of hospitalization and readmission in the US. The present study evaluated maximum diuresis while minimizing electrolyte imbalances, hemodynamic instability, and kidney dysfunction, to achieve a euvolemic state safely in a shorter period of time. METHODS AND RESULTS A protocol of combined therapy with furosemide, metolazone, and spirono...
