Aim. To study the specific features of splenic artery architectonics and evaluate results distal pancreatic resection for various tumors. Materials Methods. In anatomical part, we examined 88 organ complexes taken from people whose death was not associated with diseases abdominal organs. We studied topography dependence number its branches to pancreas on vessel tortuosity. The clinical part presents 122 resections performed during 2016-2021. 79 operations were carried out using traditional method, 32 robot-assisted, 11 laparoscopically. Results. research found a relationship between degree tortuosity pancreas, which varies three nine. Out operated patients, in 24 (19.7%) cases clinically relevant (type B) fistula that required additional treatment formed. Intra-abdominal bleeding developed 15 (12.3%) patients: early – 10, late arrosive hemorrhage 5 cases. One (0.8%) patient died. Conclusion. It is necessary consider resection. frequency formation does depend method performing operation. Of prognostic factors, body mass index statistically significant. Late develops against background fistula.
