Dietary phenotype and advanced glycation end-products predict WTC-obstructive airways disease: a longitudinal observational study

Abstract Background Diet is a modifier of metabolic syndrome which in turn associated with World Trade Center obstructive airways disease (WTC-OAD). We have designed this study to (1) assess the dietary phenotype (food types, physical activity, and habits) Fire Department New York (FDNY) WTC-Health Program (WTC-HP) cohort (2) quantify association quality its advanced glycation end product (AGE) content development WTC-OAD. Methods WTC-OAD, defined as developing WTC-Lung Injury (WTC-LI; FEV 1 < LLN) and/or airway hyperreactivity (AHR; positive methacholine bronchodilator response). Rapid Eating Activity Assessment for Participants-Short Version (REAP-S) deployed on 3/1/2018 WTC-HP annual monitoring assessment. Clinical REAP-S data consented subjects was extracted (7/17/2019). [low-(15–19), moderate-(20–29), high-(30–39)] AGE per questionnaire were assessed Regression models adjusted smoking, hyperglycemia, hypertension, age 9/11, WTC-exposure, BMI, job description. Results N = 9508 completed questionnaire, while 4015 had spirometry. WTC-OAD developed 921, 3094 never Low- moderate-dietary quality, eating more (processed meats, fried foods, sugary drinks), fewer (vegetables, whole-grains),and having diet abundant AGEs significantly Smoking not significant risk factor Conclusions successfully implemented FDNY produced valuable phenotyping. Our observational has identified low habits factors pulmonary context WTC-exposure. Dietary phenotyping, only focuses our metabolomic/biomarker profiling but also further informs future interventions that may positively impact particulate matter lung disease.
