Diagnostic performance between MR amide proton transfer (APT) and diffusion kurtosis imaging (DKI) in glioma grading and IDH mutation status prediction at 3 T

PurposeAccurate glioma grading and IDH mutation status prediction are critically essential for individualized preoperative treatment decisions. This study aims to compare the diagnostic performance of magnetic resonance (MR) amide proton transfer (APT) diffusion kurtosis imaging (DKI) in prediction.MethodFifty-one patients without were retrospectively included. APT-weighted (APTw) effect DKI indices, including mean diffusivity (MD), fractional anisotropy (FA), (MK), FA (KFA) obtained from APT diffusion-weighted images, respectively. indices tumors normalized that contralateral normal appearing white matter (CNAWM) APTw difference (ΔAPTw) between two regions was calculated. Student’s t-test, one-way ANOVA ROC analyses conducted.ResultsAmong enrolled 51 patients, 13 had glioma-II, 17 glioma-III 21 glioma-IV. 25 diagnosed as IDH-mutant, 26 IDH-wild type. MD MK differed significantly glioma-IV II/III (P < 0.05), but not glioma-II glioma-III. KFA showed no significant among three groups > 0.05). IDH-mutant group exhibited higher lower FA, ΔAPTw than type whereas comparable values. In contrast, across tumor grades with consistently better discriminatory prediction.ConclusionsAPT superior prediction, benefiting accurate diagnoses
