Dapsone, danazol, and intrapartum splenectomy in refractory ITP complicating pregnancy.
Currently there is no consensus on the treatment of refractory idiopathic thrombocytopenic purpura (ITP) complicating pregnancy. Our patient with chronic ITP complicating pregnancy, who was refractory to steroids, dapsone, and danazol, was treated successfully with intrapartum splenectomy.
متن کاملRefractory Immunological Thrombocytopenia Purpura and Splenectomy in Pregnancy
Thrombocytopenia is defined as a platelet count of less than 100,000 platelets per microlitre (mcL). Thrombocytopenia develops in approximately 6-7% of women during pregnancy and at least 3% of these cases are caused by immunological platelet destruction. Herein, we present a pregnant woman who develops at the first trimester autoimmune thrombocytopenia purpura associated with positive antiphos...
متن کاملLaparoscopic splenectomy for ITP. The gold standard.
BACKGROUND A comparison of safety, efficacy, and cost of laparoscopic splenectomy (LS) vs open splenectomy (OS) for idiopathic thrombocytopenic purpura (ITP) was performed. METHODS The records of 49 consecutive patients who underwent splenectomy for ITP (31 LS and 18 OS) at a large metropolitan teaching hospital between 3/91 and 8/95 were reviewed. Morbidity, mortality, hospital stay, operati...
متن کاملImmunomodulatory treatments for persistent and chronic immune thrombocytopenic purpura
BACKGROUND Corticosteroid sparing is required in 15% to 40% of adults with persistent or chronic primary immune thrombocytopenic purpura (ITP). Herein, the efficacy of immunomodulatory drugs (dapsone, interferon alpha, danazol, and hydroxychloroquine as second-third-line therapies in ITP is investigated. METHODS MEDLINE was searched for studies that included patients with persistent or chroni...
متن کاملDanazol in non-splenectomized patients with refractory idiopathic thrombocytopenic purpura.
Seven non-splenectomized patients with chronic refractory idiopathic thrombocytopenic purpura were treated with danazol 800 mg daily. All were glucocorticoid failures and four were refractory to all additional previous therapy. Five patients benefited from danazol and in two sustained normal platelet counts, for over 44 and 51 months, were observed. We conclude that danazol is useful for long t...
