چکیده

Aims The aim of the Scaphoid Waist Internal Fixation for Fractures Trial (SWIFFT) was to determine optimal treatment adults with a bicortical undisplaced or minimally displaced fracture waist scaphoid, comparing early surgical fixation initial cast immobilization, immediate being offered patients nonunion. Methods A cost-effectiveness analysis conducted assess relative merits these forms treatment. differences in costs healthcare system and quality-adjusted life years (QALYs) over one-year follow-up trial two arms were estimated using regression analysis. Results Our base case found that randomized had statistically significantly higher mean NHS £1,295 more than immobilization arm (p < 0.001), primarily due cost surgery. They also marginally better quality life, period, 0.0158 QALYs; however, this not significant = 0.379). combined per additional QALY £81,962, well above accepted threshold used UK internationally. probability surgery cost-effective setting only 5.6%. Conclusion Consistent clinical findings SWIFFT, results indicate scaphoid fractures, nonunion, is form treatment, resulting comparable outcomes less system. Scaphoid waist fracture.

A 35-year-old male office worker fell on an outstretched hand, injuring his dominant wrist. He had persistent pain about the radial aspect of the wrist. He reported no prior injury or symptoms proximally. On physical examination, mild swelling with tenderness was observed about the scaphoid in the snuffbox. Mild limitation of wrist range of motion existed. Anteroposterior (A), lateral (B), and ...

Hybrid Russe Procedure for Scaphoid Waist Fracture Nonunion With Deformity.

PURPOSE To assess the results of a hybrid Russe procedure using a corticocancellous strut, cancellous autologous nonvascularized bone graft, and cannulated headless compression screw to reduce the deformity reliably from a collapsed scaphoid nonunion, provide osteoinductive stimulus, and stabilize the fracture for predictable union. METHODS A hybrid Russe procedure was performed for scaphoid ...

investigating the feasibility of a proposed model for geometric design of deployable arch structures

deployable scissor type structures are composed of the so-called scissor-like elements (sles), which are connected to each other at an intermediate point through a pivotal connection and allow them to be folded into a compact bundle for storage or transport. several sles are connected to each other in order to form units with regular polygonal plan views. the sides and radii of the polygons are...

Displaced fracture of the waist of the scaphoid.

A displaced fracture of the scaphoid is one in which the fragments have moved from their anatomical position or there is movement between them when stressed by physiological loads. Displacement is seen in about 20% of fractures of the waist of the scaphoid, as shown by translation, a gap, angulation or rotation. A CT scan in the true longitudinal axis of the scaphoid demonstrates the shape of t...

Choosing a strategy for the diagnostic management of suspected scaphoid fracture: a cost-effectiveness analysis.

UNLABELLED To assess the cost-effectiveness of various strategies for the diagnostic management of clinically suspected scaphoid fracture, a decision-analytic model was built to evaluate three strategies and to compare them with a (clairvoyant) reference diagnostic management strategy. METHODS Evaluated strategies were: (A) repeated radiography up to 2 wk; (B) repeat radiography up to 6 wk; a...
