چکیده
PurposeOur purpose was to evaluate cosmetic changes after 5-fraction adjuvant stereotactic partial breast irradiation (S-PBI).Methods and MaterialsSeventy-five women with in situ or invasive cancer stage 0, I, II, tumor size ?3 cm, were enrolled lumpectomy a phase 1 dose escalation trial of S-PBI into cohorts receiving 30, 32.5, 35, 37.5, 40 Gy 5 fractions. Before S-PBI, 3 4 gold fiducial markers placed the cavity for tracking Synchrony respiratory system. delivered CyberKnife robotic radiosurgery Patients physicians evaluated global cosmesis using Harvard Breast Cosmesis Scale. Eight independent panelists digital photography 10 subdomains at baseline follow-up. McNemar tests used change cosmesis, graded as excellent/good fair/poor, from year 3. Wilcoxon signed rank subdomains. Cohen’s kappa (?) statistic estimate interobserver agreement (IOA) between raters, Fleiss’ ? IOA panelists.ResultsMedian follow-up 5, 4, years cohorts. Most patients reported both (86.3%) (89.8%). No cohort had significantly worsened by on analysis. subdomain significant worsening fair patient-physician (? = 0.300, P < .001), patient-panel 0.295, physician-panel 0.256, individual (Fleiss 0.327, .001).ConclusionsDose 30 fractions early not associated detectable is promising modality treatment cancer. Toxicity and cosmesis outcomes after single fraction partial breast irradiation in early stage breast cancer
BACKGROUND To report the clinical outcome after a Single Shot 3D-CRT PBI (SSPBI) in breast cancer patients after conservative surgery (ClinicalTrials.gov Identifier: NCT01316328). METHODS A dose of 18 Gy (in the first 4 patients) and 21 Gy (in the remaining 60 patients) was prescribed in a single session and delivered to the index area (i.e. the area of breast including the primary tumor bed ...
متن کاملPartial-breast irradiation versus whole-breast irradiation for early-stage breast cancer: a cost-effectiveness analysis.
PURPOSE Accelerated partial-breast irradiation (PBI) is a new treatment paradigm for patients with early-stage breast cancer. Although PBI may lead to greater local recurrence rates, it may be cost-effective because of better tolerability and lower cost. We aim to determine the incremental cost-effectiveness of PBI compared with whole-breast radiation therapy (WBRT) for estrogen receptor-positi...
متن کاملTreatment constraints for single dose external beam preoperative partial breast irradiation in early-stage breast cancer
Background Following breast-conserving surgery and post-operative 3D-conformal accelerated partial breast irradiation (APBI), suboptimal cosmetic results have been reported. Preoperative radiation delivery to the intact tumor enables better target visualization and treatment volume reduction. Single dose preoperative APBI has the potential to improve toxicity profiles, reduce treatment burden a...
متن کاملPartial breast irradiation for early breast cancer.
BACKGROUND Breast conserving therapy for women with breast cancer consists of local excision of the tumour (achieving clear margins) followed by radiation therapy (RT). RT is given to sterilize tumour cells that may remain after surgery to decrease the risk of local tumour recurrence. Most true recurrences occur in the same quadrant as the original tumour. Whole breast RT may not protect agains...
متن کاملAccelerated partial breast irradiation as sole radiotherapy after breast-conserving surgery for early stage breast cancer.
Authors' conclusions Based on the available evidence, the Blue Cross and Blue Shield Association Medical Advisory Panel made the following judgments about whether accelerated partial-breast irradiation (APBI) as sole radiotherapy meets the Blue Cross and Blue Shield Association Technology Evaluation Center (TEC) criteria to decrease recurrence after breastconserving surgery for early stage brea...
