Correlation between Germinal Center Differentiation and Double/Triple Hit Score in the Classification of Diffuse Large B Cell Non-Hodgkin Lymphoma: A Clinicopathological Approach

Background: Diffuse large B cell lymphoma (DLBCL) is considered the most common type of non-Hodgkin lymphoma. The global incidence DLBCL has been doubled in past decades, highlighting need for more effective treatment regimens. curability heavily influenced by a number factors, such as age, international prognostic index (IPI) score, molecular origin (COO) subtype, and presence/ absence specific chromosomal rearrangements or protein expression. Recent gene profiling studies have classified into two main categories, germinal center B-cell (GCB) activated (ABC). Validation these subtypes become applicable after immunohistochemical algorithm suggested Hans, which included three commercially available markers, CD10, BCL6, MUM-1/IRF4. In addition to this classification, models based on different sets genes markers proposed. More recently, DLBCLs with translocations MYC, along 2 (BCL2) and/or 6 (BCL6) rearrangement, are now called double-hit (DHL) triple-hit (THL), respectively. Furthermore, co-expre sion MYC BCL2 proteins without underlying new adverse indicator termed double-expressor (DEL). present study aiming combine morphological, immunophenotypical, genetic features reach reproducible subtyping. Aim: To identify relation between clinicopathological patient sex, site tumor, clinical presentation tumor stage differentiation (germinal (ABC)) double/triple hits status.Material Methods: material comprised 52 formalin-fixed paraffin- embedded blocks representative diffuse cases retrieved from archival at Pathology Department, Faculty Medicine, Alexandria University. Subclassification / post-germinal types) using panel including BCL6 MUM1 was done scoring hit expression c-MYC, also correlation their data performed.Results: there significant association (MCp= 0.018).Conclusion: Germinal status valid method evaluate prognosis DLBCL.
