Coronary artery height differences and their effect on fractional flow reserve

چکیده
Background: Fractional flow reserve (FFR) uses pressure-based measurements to assess the severity of a coronary stenosis. Distal pressure (Pd) is often at different vertical height that proximal aortic (Pa). The difference in between Pd and Pa due hydrostatic pressure, may impact FFR calculation. Methods: One hundred computed tomography angiographies were used measure differences ostia points tree. Mean heights calculate effect each artery, using correction factor 0.8 mmHg/cm. This was tested simulation intermediate stenosis give “corrected FFR” (cFFR) percentage values, which crossed threshold 0.8. Results: mean from ostium distal left anterior descending (LAD) +5.26 cm, circumflex (Cx) –3.35 right artery-posterior ventricular artery (RCA-PLV) –5.74 cm RCA-posterior (PDA) +1.83 cm. For LAD, resulted change +0.042, –0.027 Cx, –0.046 PLV +0.015 PDA. Using 200 random values 0.75 0.85, resulting cFFR clinical treatment 43% 27% 47% 15% PDA cases. Conclusions: There are significant its point normalization likely have modest on FFR, correcting for this results proportion crossing thresholds. Operators should be mindful phenomenon when interpreting values.

The relationship between coronary artery distensibility and fractional flow reserve
Discordance between angiography-based anatomical assessment of coronary stenosis severity and fractional flow reserve (FFR) has been attributed to several factors including lesion length and irregularity, and the myocardial territory supplied by the target vessel. We sought to examine if coronary arterial distensibility is an independent contributor to this discordance. There were two parts to ...
متن کاملDiastolic fractional flow reserve to assess the functional severity of moderate coronary artery stenoses: comparison with fractional flow reserve and coronary flow velocity reserve.
BACKGROUND Coronary blood flow occurs mainly during the diastolic phase of each cardiac cycle and is mainly dependent on diastolic driving pressure, especially in the left anterior descending coronary artery (LAD). We hypothesized that calculation of the ratio of the diastolic driving pressure of a stenotic LAD to its normal value, namely diastolic FFR (d-FFR), might provide precise insight int...

This paper aims to provide a short overview of the theoretical background for coronary pressure-derived fractional flow reserve (FFR) index and to focus on its clinical applicability in complex coronary intervention.
متن کاملCoronary pressure measurement and fractional flow reserve.
The shortcomings of coronary arteriography to assess the physiological significance of coronary stenoses have been recognised for decades ; therefore, the importance of additional physiological techniques is beyond doubt. One of the currently available physiological techniques is coronary pressure measurement, which has emerged over the past few years as a major step forward in the invasive ass...
متن کاملFractional flow reserve in acute coronary syndromes.
Little doubt exists about the outcome benefit of an invasive strategy in patients with an acute coronary syndrome (ACS). Yet, once the revascularization is performed, the treatment strategy of patients admitted with an ACS is not always as straightforward in daily clinical practice as indicated in guidelines. The reasons for this are at least three-fold. First, a sizable proportion of patients ...
