Complex regional pain syndrome after multiple revision total hip arthroplasty surgeries

Abstract Background Persistent pain after total hip arthroplasty (THA) is a real concern for the surgeon and significant cause of patient dissatisfaction; periprosthetic joint infection (PJI), aseptic loosening, instability are among common causes painful THA. However, few rare have been reported in literature; complex regional syndrome (CRPS) was to occur primary knee arthroplasties. Case presentation: We present female who had bilateral resurfacing when she 28 years old being diagnosed with avascular necrosis; over 16 years, satisfied. She started developing her right 2019, as revision using cementless THA performed. Six months later, PJI, which treated two stages; first-stage THA, complained persistent lower leg burning sensation change skin color, not responding conservative lines. During second-stage receiving spinal anesthesia, color limb changed normal. The postoperative diagnosis made CRPS, effectively sympathetic block. At six postoperative, pain-free, implants showed proper position. Conclusions: Although rare, CRPS could repeated surgeries, reached mainly by exclusion.
