چکیده
Colorectal cancer (CRC) is the third most prevalent malignancy and second common cause of death in US. Liver site colorectal metastases. About 13% patients with have liver metastasis on initial presentation 50% develop them during disease course. Although systemic chemotherapy immunotherapy are mainstay treatment for metastatic disease, selected predominant metastasis, liver-directed approaches may provide prolonged control when combined treatments. Hepatic artery infusion pump (HAIP) an approach which allows direct chemotherapeutic into especially useful setting multifocal When chemotherapy, HAIP improves response rate, provides more durable control, some leads to successful resection. To ensure safety, use requires multidisciplinary collaboration between interventional radiologists, medical oncologists, hepatobiliary surgeons nurses. Here, we review benefits potential risks this our single institution experience two CRC successfully treated combination chemotherapy. We recommendations adopting technique current era patient

Initial hepatic artery infusion and systemic chemotherapy for asymptomatic colorectal cancer with un-resectable liver metastasis.
PURPOSE Hepatic arterial infusion (HAI) has been proved to be an effective strategy to increase the chances of resection for colorectal cancer patients with liver metastasis (CRCLM). Herein, we aimed to evaluate the benefits and risks of initial treatment with HAI floxuridine (FUDR) and systemic XELOX in un-resectable synchronous CRCLM. MATERIALS AND METHODS HAI catheter systems were implante...

BACKGROUND Systemic FOLFOX (folinic acid (leucovorin (LV)), 5-fluorouracil (5-FU), and oxaliplatin), FOLFIRI (LV, 5-FU, and irinotecan), or FOLFOXIRI (5-FU, leucovorin, oxaliplatin, and irinotecan) chemotherapy regimens and additional molecular-target treatments, including anti-vascular endothelial growth factor, anti-epidermal growth factor receptor, and anti-multi-kinase antibodies, have been...
متن کاملAlternating hepatic arterial infusion and systemic chemotherapy for stage IV colorectal cancer with synchronous liver metastasis.
Among 41 patients with synchronous liver metastases of colorectal cancer, 15 patients underwent synchronous resection of their liver metastases and achieved a median survival time (MST) of 1,441 days (versus 748 days for the 26 patients without resection, p=0.038), a median relapse-free survival time of 652 days (MST not reached), and a recurrence rate in the residual liver of 20% (3/15 patient...
متن کاملHepatic artery infusion pump in the treatment of liver metastases.
BACKGROUND Hepatic artery infusion pump (HAIP) use in chemotherapy started in the 1960s as a way to treat liver metastases that were not amendable to locoregional therapy or surgical resection. Because of complications and limited survival benefit, the use of HAIPs fell out of favor. A resurgence has occurred in the use of these pumps, but limited information is available in the literature guid...
متن کاملImpact of liver-directed therapy in colorectal cancer liver metastases.
BACKGROUND There is a paucity of data on the current management and outcomes of liver-directed therapy (LDT) in older patients presenting with stage IV colorectal cancer (CRC). The aim of the study was to evaluate treatment patterns and outcomes in use of LDT in the setting of improved chemotherapy. METHODS We used Cancer Registry and linked Medicare claims to identify patients aged ≥66 y und...
