• Predicting clinical outcome and response to CRT in patients with HFrEF is complex. Phenomapping unsupervised hierarchical cluster analysis finds 4 phenogroups. The four phenogroups differ characteristics outcomes. terms of mortality after CRT. may help identify who will derive most benefit from approach be improved by other risk markers. Despite having an indication for cardiac resynchronization therapy according current guidelines, heart failure reduced ejection fraction receive do not consistently it. To determine whether clustering (phenomapping) can distinct differential outcomes among recipients routine practice. We used phenotypic data reduction (55 clinical, biological echocardiographic variables) define new 328 practice enrolled before therapy. Clinical rate were studied Although all met the recommended criteria implantation, phenomapping classified study participants into that differed distinctively biological, electrocardiographic Patients 1 2 had mortality, associated rates 81% 78%, respectively. In contrast, 3 39% 59%, respectively, worst outcome, a considerably increased compared phenogroup (hazard ratio 3.23, 95% confidence interval 1.9–5.5 hazard 2.49, 1.38–4.50, respectively).

Activity monitoring in heart failure patients with cardiac resynchronization therapy.

BACKGROUND Cardiac resynchronization therapy (CRT) improves functional capacity in heart failure patients. This study aimed to prospectively analyze long-term device-based monitoring of physical activity in patients undergoing CRT. METHODS AND RESULTS The Activity Log Index (ALI), calculated by CRT devices, represents the percentage of time when acceleration exceeds a threshold and monitors t...

Cardiac resynchronization therapy in heart failure patients with atrial fibrillation

Cardiac resynchronization therapy (CRT) is an important device-based, non-pharmacological approach that has shown, in large randomized trials, to improve left ventricular (LV) function and reduce both morbidity and mortality rates in selected patients affected by advanced heart failure (HF): New York Heart Association (NYHA) functional class III-IV, reduced LV systolic function with an ejection...

Cardiac resynchronization therapy in patients with heart failure: systematic review.

CONTEXT AND OBJECTIVE Cardiac resynchronization therapy (CRT) has emerged as the predominant electrical treatment strategy for patients on pharmacological therapy who present heart failure with wide QRS and low ejection fraction. The objective of this study was to investigate whether cardiac resynchronization therapy improved mortality and morbidity among patients with heart failure. METHODS ...

Ventricular asynchrony predicts a better outcome in patients with chronic heart failure receiving cardiac resynchronization therapy.

OBJECTIVES The aim of this study was to evaluate whether the clinical benefit of cardiac resynchronization therapy (CRT) can be prospectively predicted by means of the baseline evaluation of left ventricular asynchrony. BACKGROUND The reverse remodeling associated with CRT is more evident in patients with severe heart failure (HF) and left bundle branch block (LBBB) who have left ventricular ...

Sex-specific mortality differences in heart failure patients with ischemia receiving cardiac resynchronization therapy

BACKGROUND Recent studies have reported prognosis differences between male and female heart failure patients following cardiac resynchronization therapy (CRT). However, the potential clinical factors that underpin these differences remain to be elucidated. METHODS A meta-analysis was performed to investigate the factors that characterize sex-specific differences following CRT. This analysis i...
