In our case, we present a case of an 80-year-old male who was referred to the gastroenterologist for evaluation suspicious mass. CT imaging at time had shown intrahepatic and extrahepatic biliary dilations, patient thought have pancreatic or common bile duct A mass biopsy showed no malignancy, further warranted. The found elevated IgG4 levels diagnosed with IgG4-sclerosing cholangitis (IgG4-SC). has been create wide array pathologies, including autoimmune pancreatitis, dacryoadenitis, sialadenitis. These pathologies grouped under IgG4-Related Disease (IgG4-RD) category. some cases, this IgG4-RD can as subclass primary sclerosing due immune depositions swelling CBD. Due strictures caused by cholangitis, dilations might be on endoscopic ultrasound (EUS). It is imperative differentiate from malignant early recognition treatment IgG4-SC lead complete resolution. report, responded well steroid treatment.
