چکیده

Purpose To report the clinical course and treatment strategies employed in management of a case multidrug-resistant Pseudomonas aeruginosa keratitis progressing to endophthalmitis. The drug-resistant strain was later traced use contaminated EzriCare Artificial Tears multi-state cluster outbreak. Observations A 57-year-old male patient with history Descemet stripping automated endothelial keratoplasty referred for culture-positive corneal ulcer right eye that had been treated several weeks topical moxifloxacin, fortified vancomycin tobramycin, intravitreal injections His cornea cultured off antibiotics grew only rare Propionibacterium acnes. Topical steroids were reduced, but his condition rapidly deteriorated leading melt, perforation, Repeat cultures sensitivity analyses revealed growth resistant fluoroquinolones, aminoglycosides, cephalosporins, monobactams, carbapenems, intermediate susceptibility piperacillin-tazobactam. underwent therapeutic penetrating subsequently initiated on an intensive regimen chlorhexidine polymyxin-B/trimethoprim. He also pars plana vitrectomy anterior chamber washout, followed by serial piperacillin-tazobactam at dose 225 mg/0.1 mL. After 8 treatment, there gradual healing ocular surface, regression hypopyon posterior inflammation, no signs recurrent infection. public health investigation ultimately infection one cases involved multistate outbreak extensively infections Tears. Conclusions Importance Multidrug-resistant endophthalmitis caused requires consideration nonconventional antimicrobial agents experimental alternatives. are currently therapies context bacterial endophthalmitis, safe effective

Topical piperacillin/tazobactam for recalcitrant pseudomonas aeruginosa keratitis.

PURPOSE To report the treatment of therapy-resistant Pseudomonas aeruginosa keratitis with topical piperacillin/tazobactam. METHODS Retrospective report of 3 cases. RESULTS Three patients with P. aeruginosa keratitis were unresponsive to various antimicrobials. Resolution of all 3 cases of keratitis occurred upon commencement of topical piperacillin/tazobactam. CONCLUSION Therapy-resistan...

Multidrug resistant Fusarium keratitis.

CASE REPORT We report a case of keratitis in a female contact lens wearer, who developed a deep corneal abscess. The culture of a corneal biopsy scraping was positive for multiresistant Fusarium solani. The patient has a complicated clinical course and failed to respond to local and systemic antifungal treatment, requiring eye enucleation. CONCLUSION Fusarium keratitis may progress to severe ...

Effect of subinhibitory concentration of piperacillin/tazobactam on Pseudomonas aeruginosa.

Subinhibitory concentrations (sub-MICs) of antibiotics, although not able to kill bacteria, can modify their physico-chemical characteristics and the architecture of their outermost surface and may interfere with some bacterial functions. This study investigated the ability of sub-MIC piperacillin/tazobactam (P/T) to interfere with the bacterial virulence parameters of adhesiveness, cell-surfac...

Risk factors for piperacillin-tazobactam-resistant Pseudomonas aeruginosa among hospitalized patients.

Antimicrobial resistance is an emerging problem with Pseudomonas aeruginosa. This study determined risk factors for the recovery of piperacillin-tazobactam-resistant P. aeruginosa from clinical cultures from hospitalized patients. A case-control study design was used to compare two groups of case patients with control patients. The first group of case patients was defined by nosocomial isolatio...
