Cardiovascular Mechanisms of Exercise Intolerance in Older Patients with Heart Failure

چکیده

Exercise intolerance, measured by peak oxygen consumption (V̇O2), is a hallmark feature of heart failure (HF). The effect compounded in the elderly HF patient aging-associated changes such as reduction lean muscle mass, an increase adiposity, and maximal rate peripheral blood flow with exercise. There non-linear V̇O2 age that accelerates later decades life. Peak further reduced due to central maladaptation from HF. Central mechanisms include impaired rate, stroke volume, contractility, increased filling pressures, blunted vasodilatory response. Peripheral endothelial dysfunction, muscles, skeletal oxidative capacity. This review presents focused update on leading aerobic capacity older patients.

Pulmonary hypertension and exercise intolerance in patients with heart failure.

OBJECTIVES This study was undertaken to investigate the relationship between pulmonary hypertension and exercise performance in patients with heart failure. BACKGROUND The exercise capacity of patients with heart failure is frequently reduced. Pulmonary hypertension may contribute to this exercise intolerance by impairing blood flow through the pulmonary circulation. METHOD Three hundred tw...

Exercise intolerance in patients with chronic heart failure.

Patients with chronic heart failure (CHF) experience significant morbidity because of dyspnea and fatigue with activities of daily living. Although central hemodynamic abnormalities are the hallmark of this disorder, investigators have not shown a relationship between left ventricular ejection fraction or exercise pulmonary capillary wedge pressure and exercise intolerance in this disorder. Rec...

Skeletal muscle abnormalities and exercise intolerance in older patients with heart failure and preserved ejection fraction.

Heart failure (HF) with preserved ejection fraction (HFPEF) is the most common form of HF in older persons. The primary chronic symptom in HFPEF is severe exercise intolerance, and its pathophysiology is poorly understood. To determine whether skeletal muscle abnormalities contribute to their severely reduced peak exercise O2 consumption (Vo2), we examined 22 older HFPEF patients (70 ± 7 yr) co...

Exercise Intolerance in Chronic Heart Failure

xercise intolerance is a major clinical manifestation in patients with chronic heart failure (CHF). It was originally considered to be the result of a failure of perfusion to the exercising musculature and consequent early onset of intramuscular acidosis. Under this theory, cardiotonic agents were administered to improve symptoms and exercise tolerance in patients with CHF. However, the therapy...

Exercise intolerance in heart failure with preserved ejection fraction: A reappraisal of central mechanisms?

More than 50% of Americans with heart failure have preserved ejection fraction (HFpEF). Exercise intolerance is a hallmark of HFpEF, but the pathophysiology is not well understood. Diverse etiologies and incomplete mechanistic understanding have resulted in ineffective management strategies to improve the outcomes of HFpEF. Traditional therapies that have been beneficial in the treatment of hea...
