Audit of Antimanic Agents Monitoring in a High Secure Hospital

Aims Antimanic agents are effective in the management of mood disorders and other neuropsychiatric conditions such as epilepsy aggression. These medications may cause serious side-effects affect vital organs; hence, specific checks recommended before initiation for continuous use these medications. The aim audit was to check compliance with monitoring antimanic (lithium, valproate, carbamazepine) compare 2018 audit. Methods Preliminary data all patients on at Rampton Hospital from 01 December 2020 30 November 2021 were obtained records. Retrospective collected using a modified version tool. This includes patient hospital number, ward, medication date, pre-initiation tests. tests were, depending medication; full blood count (FBC), thyroid function test (TFT), liver (LFT), electrolytes urea (E&U), electrocardiograph (ECG). criteria based recommendation Maudsley Prescribing Guidelines (14th edition). A total 16 standards assessed overall. Results 98 prescribed through review period valproate-59, lithium -32 carbamazepine -7. Three valproate 3 had incomplete excluded. Therefore, 92 included final (valproate-56, lithium-29 carbamazepine-7). Pre-initiation LFT FBC 35.8% 41.5% respectively, while within 6 months 85.7% 87.5% respectively. For lithium, pre -initiation ECG 73.1%; TFT U&E 88.5% 96.2% There 100% level months, both months. carbamazepine, 50% respectively three (LFT, U&E). improvement when compared except which dropped 92% 87.5%. Conclusion Only guidelines. Lithium generally higher than stabilizers; poor valproate. general guidelines stabilizers apart needs be made popular especially carbamazepine.
