چکیده

Background: Liberation from mechanical ventilation is a cardinal landmark during hospitalization of ventilated patients. Decreased muscle mass and sarcopenia are associated with high risk extubation failure. A low level alanine aminotransferase (ALT) known biomarker sarcopenia. This study aimed to determine whether levels ALT increased failure among critically ill Methods: was retrospective single-center cohort mechanically patients undergoing their first extubation. The study’s outcome within 48 h 7 days. Multivariable logistic Cox regression were performed an independent predictor these outcomes. Results: included 329 median age 62.4 years (IQR 48.1–71.2); 210 (63.8%) at for 66 (20.1%) 83 (25.2%) failed the attempt after days, respectively. Low values more common requiring reintubation (80.3–61.5% vs. 58.6–58.9%, p < 0.002). analysis identified as ? 21 IU/L had adjusted hazard ratio (HR) 2.41 (95% CI 1.31–4.42, 0.001) 16 HR 1.94 1.25–3.02, Conclusions: ALT, established frailty, factor hospitalized simple laboratory parameter can be used effective adjunct predictor, along other weaning parameters, thereby facilitate identification high-risk

Clinical review: Post-extubation laryngeal edema and extubation failure in critically ill adult patients

Laryngeal edema is a frequent complication of intubation. It often presents shortly after extubation as post-extubation stridor and results from damage to the mucosa of the larynx. Mucosal damage is caused by pressure and ischemia resulting in an inflammatory response. Laryngeal edema may compromise the airway necessitating reintubation. Several studies show that a positive cuff leak test combi...

Association between prehospital vitamin D status and incident acute respiratory failure in critically ill patients: a retrospective cohort study

OBJECTIVE We hypothesise that low 25-hydroxyvitamin D (25(OH)D) levels before hospitalisation are associated with increased risk of acute respiratory failure. DESIGN Retrospective cohort study. SETTING Medical and Surgical Intensive care units of two Boston teaching hospitals. PATIENTS 1985 critically ill adults admitted between 1998 and 2011. INTERVENTIONS None. MEASUREMENTS AND MAIN...

Resource use and outcome in critically ill patients with hematological malignancy: a retrospective cohort study

INTRODUCTION The paucity of data on resource use in critically ill patients with hematological malignancy and on these patients' perceived poor outcome can lead to uncertainty over the extent to which intensive care treatment is appropriate. The aim of the present study was to assess the amount of intensive care resources needed for, and the effect of treatment of, hemato-oncological patients i...
